60,722Position Summary:The Behavioral Health Clinician is
responsible for assessing and responding to requests for mental
health or substance use/addictions counseling for people living
with HIV/AIDS and as well as other underserved individuals. This
position helps manage group-counseling services including oversight
of volunteer facilitators, scheduling, group admissions,
advertising and evaluation.COVID-19 Information: It is an essential
function and business necessity of the job that all employees are
vaccinated and that all new hires will be required to be vaccinated
within four weeks of hire. CHN requires proof of vaccination as
this is a business necessity of the job. CHN will discuss
reasonable exemptions for medical and religious purposes. If CHN

Functions:Support integrated careProgram development for integrated
care programProvide patient navigation and care coordination
through assessment, screenings, eligibility of insurance and
education as needed.Provides individual (short-term) mental health
and substance use/addictions counseling or other appropriate
interventions in-person and via tele-therapyProvide drop in brief
harm reduction counseling during assigned timesIn conjunction with
other Behavioral Health clinicians and other agency staff, provides
crisis coverage in by referral or direct intervention as
appropriateSupports the upkeep, tracking and monitoring of client
and patient activities in agency databaseIn conjunction with other
Behavioral Health staff, maintains network and referral system of
prevention, mental health, and addiction treatment options
available for clientsDemonstrates, abides by, and helps enforce
HIPAA regulations amongst staff and departments of agencyActs as a
liaison between clients, patients and all relevant professionals,
programs and resourcesParticipates and attends staff, department,
team and community meetings upon requestPerforms effectively within
organization and interpersonally with co-workersParticipates in
fund-raising and advocacy events on behalf of the agencyCoordinate
and maintain an active caseload for new and existing clientsPreform
assessments, plan creation, and assist patients with access to
medication and adherence to treatment as wellas risk reduction
plansResponsible for tracking, monitoring and occasional reporting
of patient activities, including access adherenceComplete necessary
paperwork and data entry in a timely fashion including entering
case notesProvide appropriate patient referrals to both internal
and external agenciesCollect necessary documentation required for
program complianceAssist patients as they move through the service
delivery systemAssist with the elimination of barriers including,
but not limited to: insurance, financial housing and
transportationAct as a liaison between patient and all relevant
professionals, programs, and resourcesEducate patients and others
about HIV disease process and preventionAssist with, and
coordinate, community education and outreach initiatives as
requestedFacilitates group counseling services in-person and via
tele-therapyParticipates and attends staff, department, team and
community meetings upon requestBe willing to commute between
offices on a set schedule basesPerforms effectively within

assigned.Qualifications:Minimum one-year experience working with
individuals, couples, families, and groups. Must have experience or
knowledge regarding HIV and/or other chronic health conditions.
Experience working with diverse populations including LGBTQIA2+
populations. Experience providing substance use/harm reduction
counseling. Ability to speak, read, and write in both English and
Spanish preferred.Education:Masters Degree in Counseling or Social
work required. Active Colorado LAC, LPC or LCSW license strongly
preferred. Candidates working towards LAC, LPC, or LCSW licensures
with a registration or certification with the Mental Health Section
of the Department of Regulatory Agencies and relevant experience
may be considered. Must be willing to work towards licensure
fulfillment for Certified Addictions Specialist (CAS or
LAC).Software Utilized:Experience working with basic office and
database computer programs, including familiarity with Microsoft
Outlook, Word, Excel, Access, SharePoint and the
internet.Knowledge, Skill and Ability:Must have an applicable
understanding and experience with the harm reduction model, stages
of change model, and motivational interviewing. Excellent client
relations and management skills. Strong verbal and written
communication skills. Proven organizational skills with high
attention to detail. Ability to work well within team environment
and overall organizational structure.Material and Equipment
Directly Used:Works with standard office equipment including
computer workstation, printers, copiers, scanners and fax machine.
Works with word processing software, spreadsheet applications, and
presentation software. Communicates with clients and vendors using
the telephone, e-mail, the Internet, and fax.Physical Demands and
Work Conditions:Works in an office environment. Requires clear
speaking ability and the ability to use a telephone for extended
periods. Requires ability to view a computer screen (near acuity).
Requires fingering agility for keyboarding and using a telephone
